The case measures 35.2 millimeters in diameter and 8.65 millimeters in thickness. It is fully paved with 1,500 brilliant-cut diamonds (totalling 6.53 carats) and 876 rubies (totalling 6.58 carats) using the snow-setting technique, which creates a seamless, uninterrupted sparkle across the surface of the watch. The watch features a glare-resistant sapphire crystal and a transparent sapphire case back that allows a view of the inner workings of the watch. The Nautilus Ref. 7118/1452G is water resistant to 30 meters.

The beating heart of the Nautilus Ref. 7118/1452G is the self-winding 26-330 S caliber. This mechanical movement is composed of 181 parts and vibrates at a frequency of 28,800 vph (4 Hz).
